You can access request forms under More Services in Online Banking. Some forms (ie. dispute transactions, make overdraft decisions, and request loan payoff quotes) are available in "Self-Services Center" under the Service tab in the Republic Bank Mobile app.
Update your address, phone number, or email, add travel notes to your debit or credit card, request a new debit card, make overdraft coverage decisions, download a direct deposit form, and more.
